We are delighted to confirm that Dr Mark Johnson from Buckinghamshire Health Care Trust, will be speaking about the work that he and the BHT team have been undertaking to identify and treat patients with high cholesterol and improve cardiovascular health across their population.
BHT (Buckinghamshire Health Care Trust), part of Buckinghamshire, Oxfordshire and Berkshire West ICB (BOB) use the TVS (Thames Valley and Surrey) single consolidated Shared Care Record platform incorporating Population Health.
As part of BHT’s vision to promote healthy communities, the Live Well project was set up to improve cardiovascular health across Buckinghamshire. The project, led by Andrew McLaren and Dr Mark Johnson is looking to rapidly access and treat patients with a high LDL (cholesterol LDL-C) and who have previously had a cardiovascular event. LDL-C is proven to have a significant impact on cardiovascular health and reducing it is proven to lower the incidence of cardiovascular events.
New Lipid lowering therapies are now available and BHT were looking for a Population Health Dashboard to identify those patients who may benefit from these new NICE approved therapies
Using Graphnet Health’s Population Health Platform we have used GP data and Johns Hopkins algorithms to develop a dashboard that identifies patients with high levels of LDL-C, including details of comorbidities, medications, test results etc.
The team at BHT are using the dashboard to identify and contact patients with high LDL-C, with a view to offering them what are potentially lifesaving therapies.
