'Data rich but information poor' is a phrase regularly attributed to the NHS, however the power of a curated linked patient data set from across health and care can provide Integrated care systems with the opportunity to improve patient care through the use of shared care records and population health analytics. Graphnet ensures that a care community can turn data health, into information shared.

Graphnet's Shared Care record sharing platform (Highway) builds a linked patient record created from National spine demographics, primary care data, Hospital episodes, community, mental health, social care, ambulance, out of hours and hospice data. Population health users can also link in wider determinants data, PHE fingertips, secondary users service data sets and waiting list data sets, as required.

Areas of Care

GP data. All aspects of coded information is made available from the GP system to support the treatment of the patient.
Acute. Full HL7 realtime data feeds available for the interactions with Acute services ensures that each care provider is able to see what has and will be happening with patients which they provide services.
Social care. As part of a truly integrated care record, the access to Social Care feeds gives a truly holistic view of the person. Providing access to both Children's and Adult's services allows the care community to see interactions which have a direct and real impact to the treatment of the citizens included.
Mental health. Mental Health combined with outpatient and inpatient data sets provides a complete picture for patients. Mental Health can be combined with outpatient and inpatient data sets. The ability to see both MHA and CPA status combined with risk / plans means that the care community is able to provide the service needed to the user.
Community. Community care can combine with both Inpatients and outpatients. Giving a full view of the interaction with community services within the areas allows the service to flag patients who are engaged and give details of what is taking place.
Out of hours. Unscheduled care episodes are an everyday part of the services provided. All care providers are able to see details of what has happened to the patient based on both legitimate relationship and role of the viewer.
Ambulance. Emergency teams have real-time access to patient data, enabling them to make more informed decisions regarding care, including if there has been an escalation in the in the disease process, and how how many times the emergency services have been called for assistance.
Hospice. Provides a view of the interaction with the person during their stay in the service.

Generic Data Feeds, Covering All Areas of Care

Laboratory. Regardless of point of collection data can be displayed for all test results collected outside of the GP systems.
Radiology. Providing the textual context of radiology clinical staff are kept informed of the state of the examination and its result.
Medication. Medication feeds allows the details of medication received during inpatient episodes of care.
Cancer. A specialist data feed providing a detailed insight to treatment being received.
Child Health. A feed providing details of the interaction with children within the care community.
Referral. Available to all care settings.
Allergy. Available to all care settings. Allergies of any type maybe fed to the system and displayed when the patient is accessed, increasing patient safety.
Alerts. Available to all care settings. The alerts maybe defined by the organisation sending the feed. The Alert is flagged to each user that accesses the patient.
CGL - Alcohol & Substance Misuse. A specialist view of the CGL supports the treatment of the patient.
Maternity. Providing details of both mother and baby, this data set is provided in real-time and gives a comprehensive overview.

GP Connect

Graphnet has developed an interface to GP Connect and is in the process of being accredited by NHS Digital. We can now make this new and exciting development available in your Shared Care Record.

What does GP Connect do?

GP Connect supports Graphnet users to access a real-time view of a patient's GP record, even when that patient is registered with a GP practice outside of the ICB and, as such, does not have a record on the local CareCentric Shared Care Record.

The patient in question must be registered on one of the existing source systems that feed the Shared Care Record in your ICB/region, although that would happen with most new encounters.

One example of this is where a patient that is admitted to Accident & Emergency or an Urgent Treatment Centre and registered on their system (where that system is connected to the Shared Record).

GP Connect Workflow

“GP Connect helps clinicians gain access to GP patient records during interactions away from a patient’s registered practice and makes their medical information available to appropriate health and social care professionals when and where they need it, to support the patient’s direct care.

From a privacy, confidentiality and data protection perspective, GP Connect provides a method of secure information transfer and reduces the need to use less secure or less efficient methods of transferring information, such as email or telephone.” NHSE [1]

Graphnet has connected to the GP Connect Access Record Service. At this time we are not offering
GP Connect Send Document functionality or access to the Appointment Management Service.

GP Connect: Access record

GP Connect: Access Record allows authorised clinicians to access GP patient records held on their practice system. Access Record has two methods of retrieving information:

1. Access Record: HTML enables a read only view of a patient record which can be viewed within another care setting via
an accredited system or application. The way this information is displayed is controlled by NHSE.

2. In the future, Graphnet hopes to develop the GP Connect integration further to enable the structured data format. This will provide more flexibility in terms of the user experience (UX) and ability to structure data in the record more efficiently.
