New way of working’ as Doc Abode and Docobo work together to support efficient healthcare delivery and enhance patient safety

Doc Abode and Docobo Remote Monitoring are working together to keep patients out of hospital and keep care closer to home, by creating an interface to join up their remote monitoring and real time clinician deployment systems.
The collaboration was formed to benefit patients and to support clinicians in a more proactive way, with remote monitoring and helping to provide 24/7 care. The new output will benefit a wide range of care providers, from: ICSs, STPs CCGs, PCNs to GP Federations, social enterprises and out of hours care providers, as it helps to support the health system to deliver care more efficiently while increasing the capacity of the workforce and improving patient outcomes.
A care provider might need to keep a patient safe at home with more intensive monitoring after a hospital discharge, in order to prevent readmission. The interface between Doc Abode and DOC@HOME will enable an alert from the Docobo Remote Monitoring system to go straight to clinicians or a clinical hub through one platform. The GP will have visibility of trending data and a secure system to respond to it and react to any changes in patient status. This new concept and flexible way of working fits in with patients and care providers alike, providing more proactive care with one secure app for multiple organisations. It supports continuity of care and is aligned with priorities within the NHS England Long Term Plan, as it will keep more people out of hospital and in the community and keep patients safer.
