Graphnet’s National User Group 2023

David Grigsby, Chief Revenue Officer, provides an overview of Graphnet’s national user group, which took place virtually on 15 June 2023.

On 15 June, Graphnet held its annual national user group – virtual this time, though a get-together in-person might be on the cards for the next one, depending on customer feedback. The user group was a great opportunity to get Graphnet and all our clients together, to share thoughts, ideas, how various initiatives at Graphnet are progressing and to hear some inspirational customer success stories. The feedback from attendees has been really positive with comments including:

I thought I’d share an overview of the day, and some of our customers and presenters, who did such a good job with their updates and examples of how the Graphnet solutions are being used.

The day kicked off with an introduction from our co-Chairs, Tracie Tiller, who is Informatics Programme Manager at the University Hospitals Dorset NHS Foundation Trust, and Mark Wright, CTO at the NHS Greater Manchester Integrated Care Board.

I then provided a company update, which included announcing the completion of our move to EMIS X-Analytics' (EXA) for providing GP data to integrated care systems and care communities. I talked a little about the benefits, including the fact that amount of data has increased, and is SNOMED coded, which will give Graphnet greater flexibility in the future to provide an enhanced user experience to care teams using CareCentric products.

We went on to share some of our new developments and the CareCentric and Population Health roadmaps, which were well received and generated some interesting questions after the presentations had taken place.

Our first customer presenter was Julia Riley, clinical lead and palliative care consultant at Coordinate My Care (CMC), a clinical service that enables patients to start their own end of life care plan online via myCMC and then to have the plan completed by a doctor, nurse or other healthcare professional. CMC was first established in London in 2012 and then rolled out to NHS South, Central and West (SCW) in late 2022, which is the project that Julia focused on.

Julia explained how end of life care plans are developed with patients and then made available to all health and care workers, including emergency services, through the shared care record, which is powered by Graphnet – and she also discussed how the analysis of population health data helps health and care teams identify which people are most likely to need a care plan within the coming year. She talked us through some real-life examples, which brought home how the digitally led approach has helped people achieve the final days that they really wanted and made things just a little bit easier for their families as well. Graphnet solution owner, Lesley Harvey, then led a demonstration to show how CMC has been integrated into CareCentric.  

The next presentation saw Tom Muller, customer relationship executive at Docobo and Anna Fishta, programme lead at Frimley Health and Care Integrated Care System (ICS) discussing the remote monitoring platform, DOC@HOME and various projects that they have been deploying over the past 18 months. Anna covered three areas, starting with how Frimley uses population health analytics, provided by Graphnet, to proactively target patients and cohorts for remote monitoring. She then went on to explain the remote monitoring tools that enable patients and care home residents to enter information about their health conditions; and finished by looking at the shared care record, and the benefit of having all resident details visible to key health and care professionals.

Tom then took the reins and talked everyone through how some of the virtual care pathways were implemented, including the remote pulse oximetry service that was set up in response to Covid, where around 3,000 patients were onboarded within the space of 2-3 months. Also, the care home project that was launched to improve how care homes escalate the care of deteriorating residents to local healthcare providers.

It was fascinating to hear some detail about how this health and care ecosystem has been developed in quite a short time – and I’m sure we’ll hear a lot more from this team over the coming months as new use cases are developed.

Next was our population health consultant, Andy Evans with a demo and presentation on elective care. Among other things, he looked at how Graphnet’s population health tools are improving waiting lists (including through identifying duplicate and other incorrect information) and supporting perioperative care.

The user group finished with our chief business development officer, Andy Bratt taking attendees through data feeds and access, including some feeds that are live and others that are going through the quality assurance process. Many of the new additions have been in direct response to requests from our customers and will make a tangible difference to the effectiveness of their shared care and population health solutions from Graphnet. We’re looking forward to sharing some of those a little more widely and publicly when we can.

We ended with a Q&A, which our presenters were able to respond to.
